Y'know, funnily enough, you can die while randomly rolling a PC in Eclipse Phase. However, due one of the vital technologies of the setting where the mind can be stored and restored from a digital backup (Almost directly taken from Altered Carbon), this doesn't end your character, just gives them a few bumps in their life.
